The Self Care Team had produced this guide to support NHS Salford meet its vision and in response to wishes of Salford People and Health Care Professionals who identified the need for this resource.

NHS Salford is a forward-looking and ambitious organisation dedicated to improving the health of local people.

We are responsible for:

Improving the health of people who live in Salford.
Developing and providing primary and community health services.
Securing the provision of high quality services from local hospitals and other care providers.

We recognise that to deliver these functions effectively we need to work in partnership with other organisations, including local hospitals and the City Council. We also have a key role to play in the regeneration of the city and are committed to being 'Your Leader for Health IN Salford'.

We have developed a long-term vision that:
